Patients: refer yourself

Complete the form below with your details and the reason you want to be seen.
Tell us if your GP has already referred you or given you a letter.
We call you to confirm the consultant, the clinic and the earliest appointment.

How do GPs refer to Cervino Medical?

Find Cervino Medical on your practice management system and send the referral through Healthlink, marked for the specialty or the consultant you want. You can also use the referral form on this page and attach or paste the referral letter.

Can patients refer themselves?

Patients can contact us directly using the referral form or by phone. A GP referral is still needed for most consultations and for insurance, and we will help you arrange one if you do not have it.

What happens after I send the form?

The clinic team reads every referral within one working day, checks the specialty and urgency with the consultant, and contacts you by phone or email to offer an appointment. Neurology patients are seen within two weeks of referral.
